By using our online Script Renewals, you are agreeing to be bound by the following terms and conditions.
1. For urgent prescriptions, please call Hyde Park Medical Centre on 92831234. An alternative is to utilize our Online Script Renewals service which requires 48 hours advanced notice. Please allow extra time for weekends and public holidays, postage and handling and possible technical problems.
2. The fee for a repeat prescription is $30.00 through PayPal or Visa/Master Card. Repeat prescriptions involve numerous clinical and administrative steps therefore a minimum of $12 will not be refunded if your script is unfortunately declined for renewal by the doctor. Additional $2 fee will apply if script is required to be posted or faxed.
3. All requests are reviewed by a doctor and a decision is made to determine whether a repeat prescription request is appropriate for your health care needs and is subject to the doctor discretion.
4. A repeat prescription request does NOT guarantee that a script will be written for you successfully.
5. A Successful online script will be informed or notified by our staff using either SMS, Email or Telephone call
6. A NON successful Script - A minimum refund of $18 will be issued back into your PayPal account.
7. We shall not be held responsible for any conflicts or errors resulting in use/misuse of this service, including but not limited to; any negative experiences or outcomes resulting from the online Script Renewals.
8. By communication electronically with Hyde Park Medical Centre, you release the Practice from all claims, losses, expenses and liabilities caused by any of the risks of using our online services.
9. HPMC online Script is a convenient service for time-poor patients with straightforward, non- emergency medical needs. Please note, HPMC online-Scripts is not available for new medicine requests or emergency conditions.
10. If you decide to cancel your request, a full refund will not be provided. However, a partial refund will be considered at our discretion.
